There’s a new collaborative partnership between AFC Urgent Care and the local hospitals that serve Wilton and Fairfield County.
American Family Care (AFC), a leading provider of urgent care and occupational medicine, has forged an agreement with Western Connecticut Medical Group (WCMG) and its affiliated hospitals–Norwalk Hospital, Danbury Hospital and New Milford Hospital–to facilitate coordinated patient care.
Through this collaboration, patients from AFC’s nine Fairfield County locations who require follow-up medical care and wish to have WCMG provide that care will have improved access to primary and specialty care practices within WCMG. Additionally, WCMG patients who are not suffering a medical emergency, but require treatment during hours when WCMG’s offices are not open–such as nights, weekends and holidays–will be made aware of the services available at AFC’s urgent care clinics. For those WCMG patients who are treated at an AFC clinic, AFC will provide follow-up information back to the patient’s WCMG physician to optimize care coordination.
“This relationship is completely aligned with our patient-centric approach to health care,” said Tom Kelly, AFC’s local area executive. “It also signals to patients that, no matter what time of day or day of the week, the brands they already know and trust are ready to provide quality treatment across a broad spectrum of their health care needs.”
AFC’s urgent care clinics provide patients with immediate access to treatment of illness and injury. Additionally, AFC provides services for occupational health, workers comp, travel medicine, vaccinations, wellness visits and various types of physicals, such as annual, sports, camp and school.
The AFC Urgent Care clinic closest to Wilton is located just over the Norwalk border at 607 Main Ave., near the DMV.
In addition to providing primary care, WCMG offers a wide range of specialty professional medical services, including surgical services, such as cardio-thoracic, colon and rectal, vascular, general surgery, and wound care, among others. WCMG specialty care physicians meet patient needs for advanced diagnostic and treatment services through WCMG’s affiliated hospitals, Danbury Hospital, New Milford Hospital, and Norwalk Hospital.
